Concrete firm fined £100,000 following university fatality
A concrete-products manufacturer has been ordered to pay £240,000 in fines and costs following the death of a carpenter during construction of a new accommodation building at Bath University.
Creagh Concrete Products Ltd was contracted to design, manage and build the concrete framing for 380 new student rooms at the University’s Claverton Down Campus.
The company sub-contracted Philip Hames to work as a carpenter and on 1 November 2007 he and a colleague were attempting to install wooden shuttering on the first floor of the building.€

Again no refernece o the requirements of CDM to assess the risk at design stage, and thereafter to adopt adequate control of said risk.
Where are the appointed people whom determine the design risk?
What of the CDMC requirement to ensure design risk is mitigated by adoption of a proven SSoW?
Who designed the propping detail, and where is the reference to the safe sequence of work? Any propping detail generally requires adequate head restraint, this is not rocket science.
Truely shocked.
