Manchester Arena attack: Risk assessment was ‘pretty much box ticking’
An inquiry into the Manchester Arena terror attack has been told that the risk assessments carried out by operators were ‘flawed’ and ‘pretty much box ticking’.
In 2017, a suicide bomb killed 22 people attending an Ariana Grande concert at the Manchester Arena. The inquiry into the attack has been told that no specific risk assessment was carried out for that specific concert and no consideration was given to the threat or a terror attack.
Miriam Stone, SMG’s Head of Events and one of the duty managers on the night of the attack, said that while there were flaws with the written documentation, “we did assess the risk”.
When asked by Paul Greaney QC, counsel to the public inquiry, whether the assessments were done to ensure a ‘box was ticked’, she replied: “I think we had got to the point where that is pretty much how it got used.”
The inquiry continues.

Same old, same ole story of complacency and expediency just like Occupational Health of DSE operators over the last three decades accepting in the 90’s no one noticed that their induced work related visual repetitive stress injuries, in the chain f causation of other MSD’s, closely mirrored those with preexisting visual impairments / disabilities in “Accessing Text” being addressed by Abilitynet if, no one else did at that time.
The 1998 PUWER regs offered an opportunity to at least mitigate the predictable harm and visual injuries to operators just, little or no attempt to offer guidance even following the HSE RR 561 2007 nor following Safety Alerts in 2017 recognising 58% of operators at risk including the visually impaired and Dyslexics only now as the “Accessibility” nudge campaigns “Kicks-Off” supported by both earlier WCAG 2.1 and with the 2019 ISO 30071.1 DSE Interface Colour Contrast Calibration Standards the penny will drop that 20% lost productivity maybe recovered impacting on the exponential rise in presenteeism?

Human Factors and expediency of “tick-boxing” not something uncommon from individuals carrying-on regardless self-harming to schools, institutions and business right up to Government.
Not the first disastrous loss of life in the public domain nor in industry where either complacency or fatigue factors in the chain of causation.
