Recycling plant had catalogue of safety failings

A factory worker’s arm was severed while he was clearing a blockage from a conveyor at a recycling plant in Scunthorpe.
The 25-year-old man was operating a fragmentiser, which formed part of the metal sorting line, when the incident took place at City Scrap Ltd’s facility on 8 March 2011.
He was working at a picking station on the conveyor to remove pieces of copper when he noticed something was caught in the belt. He tried to clear the blockage by using a stick, which was standard practice at the site, when his right arm was drawn into the machine and severed at the shoulder. He has been unable to return to work owing to his injuries.
An investigation by the HSE uncovered multiple safety failings at the plant. There was an absence of adequate guarding around the moving parts of the machine and no emergency-stop buttons on processing lines. There was no safe system of work in place and staff weren’t adequately trained to operate the machine.€
